Exercise training in COPD: muscle O2 transport plasticity

چکیده

Both convective oxygen (O 2 ) transport to, and diffusive within, skeletal muscle are markedly diminished in patients with COPD. However, it is unknown how these determinants of peak O uptake ( V ′ mO respond to exercise training Therefore, the purpose this study was assess plasticity Adaptations 8 weeks single-leg knee-extensor were measured eight severe COPD (mean± sem forced expiratory volume 1 s (FEV 0.9±0.1 L) healthy, well-matched controls. Femoral arterial venous blood samples, thermodilution-assessed leg flow used determine utilisation at maximal pre- post-training. Training increased both (by ∼26% from 271±29 342±35 mL·min −1 controls ∼32% 418±37 553±41 ), restoring only ∼80% pre-training control . Muscle similarly ∼38% 6.6±0.9 9.1±0.9 ·mmHg ∼36% 10.4±0.7 14.1±0.8 reaching ∼90% values. In contrast, significantly 688±57 865±69 leaving (438±45 versus 491±51 ∼70% While largely restored by training, remained constrained limited transport.

Muscle metabolism and exercise tolerance in COPD.

This article explores the hypothesis that dyspnea in patients with COPD arises from an imbalance between the load placed on the respiratory muscle pump and its capacity. Evidence to support this concept is presented, and possible therapeutic approaches are discussed.

High-intensity inspiratory muscle training in COPD.

The aim of the present study was to investigate the effects of an interval-based high-intensity inspiratory muscle training (H-IMT) programme on inspiratory muscle function, exercise capacity, dyspnoea and health-related quality of life (QoL) in subjects with chronic obstructive pulmonary disease. A double-blind randomised controlled trial was performed. Sixteen subjects (11 males, mean forced ...

Inspiratory muscle training in COPD patients.

This is a review of physiological and clinical aspects on training of the respiratory muscles in chronic obstructive pulmonary disease (COPD) patients. Three types of exercise training have been used: strength, endurance, and co-ordination. With each type, intensity of the stimulus, specificity and the proper evaluation of the training are important. The rationale for training respiratory muscl...

Inspiratory muscle training improves breathing pattern during exercise in COPD patients.

Dyspnoea is typically the main symptom limiting exercise capacity in patients with chronic obstructive pulmonary disease (COPD) [1–3]. Exertional dyspnoea has been linked to dynamic hyperinflation (DH), when lung expansion critically encroaches upon the inspiratory reserve volume (IRV) [4]. Consequently, patients develop a rapid and shallow breathing pattern, which is energetically opposite to ...
